About this role

Under the general administrative supervision of the Chief Pharmacist, the Clinical Pharmacist – Advanced provides direct pharmaceutical care within the outpatient pharmacy program. Works in close collaboration with the Pharmacy Clinical Coordinator for clinical guidance on services, protocols, and program development. Serves as a subject matter expert in complex medication management and chronic disease states.

Provides comprehensive pharmaceutical care with expanded clinical responsibility, supporting routine pharmacy operations as needed. Practices with a high degree of autonomy under approved protocols and collaborative practice agreements. Monitors, titrates, and adjusts medication therapy within established guidelines.

Documents clinical activities, updates medication lists, and maintains allergy information in medical records. Serves as a therapeutic consultant and clinical resource to medical staff. Collaborates closely with pharmacy, nursing, and medical teams while upholding confidentiality of privileged information.

Provides formal and informal education, in-services to staff, and training support for pharmacy residents, students, and medical students. Offers guidance, training, and mentoring to Clinical Pharmacists. Prepares and delivers educational lectures and presentations as requested.

Requirements

  • Doctorate/Professional Degree
  • Experienced level in clinical pharmacy
  • Ability to practice with high degree of autonomy under approved protocols and collaborative practice agreements
  • Expertise in complex medication management and chronic disease states
  • Proficiency in performing focused physical assessments for pharmacy services like anticoagulation management
  • Skill in ordering and interpreting laboratory tests per protocols

Responsibilities

  • Obtains and documents relevant medical, medication, and health histories; follows established treatment plans; and monitors patient response to therapy
  • Performs focused physical assessments related to pharmacy-based clinical services (e.g., anticoagulation management) and identifies normal versus abnormal findings
  • Orders and interprets laboratory tests as authorized by approved protocols and collaborative practice agreements
  • Monitors, titrates, and adjusts medication therapy within pharmacy clinics and established protocols
  • Provides therapeutic drug monitoring services and evaluates medication regimens for safety, effectiveness, appropriateness, and cost
  • Serves as a therapeutic consultant and clinical resource to medical staff
  • Counsels patients on medication use, storage, precautions, and potential side effects
  • Provides guidance, training, and mentoring to Clinical Pharmacists
