Sentara Health Plans is hiring a Clinical Pharmacist for the Health Plan-Remote. This full-time permanent position requires 40 hours weekly with standard hours from 8am to 5pm EST, Monday through Friday. Remote opportunities are available in states including Virginia, North Carolina, Alabama, Delaware, Florida, Georgia, and many others.

Demonstrates comprehensive knowledge of pharmacotherapeutic principles, medical terminology, pathologies, disease state and population management, age-appropriate therapy, and pharmaceutical care management. Works with Sentara Health Plan members to close medication-related gaps in care and handles referrals from the team. Provides counseling to members on their medication therapy.

Requirements

  • Pharmacist License (RPh) required; will consider new graduated pharmacists and those not licensed in Virginia due to relocation
  • Must obtain Virginia license within 90 days of start date
  • Experience with online pharmacy adjudication systems required
  • Experience with HEDIS or Managed Care Pharmacy or Pharmacy Benefits Management (PBM) highly preferred
  • Advanced certification in HIV, oncology, infectious disease, or ambulatory care preferred

Responsibilities

  • Demonstrate comprehensive knowledge of pharmacotherapeutic principles, medical terminology, pathologies, disease state and population management, age-appropriate therapy, and pharmaceutical care management
  • Work with Sentara Health Plan members to close medication-related gaps in care
  • Work on referrals from the team
  • Counsel members on their medication therapy
  • Utilize online pharmacy adjudication systems to manage pharmacy benefits
  • Apply population health management strategies to improve member outcomes
  • Support managed care initiatives through clinical review and intervention
