About this role

The Clinical Pharmacy Specialist supports patient care responsibilities for specialty pharmacy patients. Serves as a liaison between the Specialty pharmacy staff and the Assistant Director of Pharmacy. Delivers comprehensive services for patients receiving specialty medications across access, delivery, and innovative clinical care.

Applies clinical knowledge and expertise of specialty disease state management to provide patient assessment, medication review and reconciliation, patient education, and regular follow-up. Develops individualized patient care plans and performs therapy management baseline and periodic reassessment prior to medication dispensation. Involves adherence monitoring, drug interaction assessment, side effect management, and comprehensive medication reviews.

Provides functional supervision of technical staff, oversees daily operations, and manages time efficiently for high productivity. Supports providers in selecting specialty medications, responds to drug information requests, and writes insurance coverage appeal letters. Ensures optimal effectiveness and safety of medications dispensed to healthcare teams and patients.

Contributes to accreditation maintenance, quality improvement initiatives, and participates in the Quality Management Committee. Develops educational topics, staff development programs, and medication protocols; maintains clinical pharmacy rotation for students and residents. Assists with policies, procedures, performance reports, and mentoring scholarly activities.

Requirements

  • Clinical knowledge and expertise of specialty disease state management including oncology
  • Ability to perform patient assessment, medication review and reconciliation, and patient education
  • Proficiency in adherence monitoring, drug interaction assessment, and side effect management
  • Experience conducting comprehensive medication reviews and patient counseling
  • Working knowledge of all State and Federal laws and regulations affecting the practice of pharmacy
  • Skills in writing insurance coverage appeal letters for specialty medications
  • Familiarity with specialty pharmacy management software for documentation
  • Ability to provide drug information to healthcare teams

Responsibilities

  • Develops individualized patient care plans and performs therapy management baseline and periodic reassessment for every enrolled patient
  • Supports providers in selecting appropriate specialty medication(s), responds to patient-specific drug information requests, and writes insurance coverage appeal letters
  • Ensures optimal effectiveness and safety of medications dispensed and provides accurate, clear, and timely drug information
  • Provides functional supervision of technical staff and oversees daily operations to maintain high productivity
  • Contributes to accreditation maintenance and renewal along with quality improvement initiatives
  • Develops and presents educational topics, staff development programs, and medication protocols
  • Assists the Assistant Director of Pharmacy in developing new policies and procedures
  • Maintains a clinical pharmacy rotation for students and residents and supports resident research
