About this role
This is a specialist position. The Oncology Clinical Pharmacist Specialist is responsible for the development, implementation, and documentation of chemotherapy services in the inpatient and outpatient settings. This pharmacist coordinates the oncology service according to the most recent guidelines and recommendations published in pharmacy and medical literature.
Directs and initiates development, implementation, and maintenance of clinical pharmacy services. Provides clinical consultation to the Medical Staff and other health care providers, suggesting appropriate cost-effective therapeutic alternatives. Communicates and documents all activities and interventions, including medication order changes, pharmacokinetic dosing, monitoring, adverse drug reactions, and errors.
Collaborates with all health care providers to ensure timely resolution of concerns and questions. Assists hospital personnel in the development of clinical pathways, protocols, policies, and procedures. Fosters teamwork and takes responsibility to ensure delivery of high-quality patient care.
Actively participates on clinical committees including pharmacy and therapeutics committee. Provides continuous education to students, residents, pharmacists, and other healthcare professionals. Demonstrates awareness of JCAHO, CMS standards, State Board of Pharmacy requirements, and departmental policies.

Requirements
- Pharm.D. Degree with 2-3 years of experience in inpatient pharmacy hospital/clinical capacity or completion of Pharmacy Residency
- PGY1 Residency Program; required
- PGY2 in Oncology, preferred
- Currently licensed or registered to practice pharmacy in Louisiana
- Board-certified oncology pharmacist (BCOP), preferred
- Demonstrates awareness and understanding of documenting information in the medical record, JCAHO and CMS standards, and State Board of Pharmacy requirements
Responsibilities

- Suggests appropriate cost-effective therapeutic alternatives as needed
- Communicates and documents all activities/interventions including medication order changes/clarifications, pharmacokinetic dosing/monitoring, adverse drug reactions and errors
- Fosters teamwork and takes responsibility to ensure delivery of high-quality patient care
- Collaborates with all health care providers to ensure timely resolution of concerns and questions
- Provides ongoing education and timely drug information to physicians, nursing, pharmacists, and other healthcare professionals
- Facilitates the development and implementation of treatment guidelines, pathways, protocols, formulary changes, and medication use evaluations
