About this role
PAC pharmacists work in pre-anesthesia clinics across multiple acute care campuses, including 39th & Rainbow (Bell Hospital Clinic), Indian Creek Campus, and potentially Olathe Health campus. They provide care to patients in clinics and via telehealth, primarily on 8-hour shifts with occasional 10-hour shifts. They support medication planning prior to surgeries and coordinate care across providers for safe preoperative medication use.
PAC pharmacists actively participate on multidisciplinary teams by attending patient care rounds and monitoring drug therapy for safe, cost-conscious regimens. They complete patient-specific medication profile reviews, executing protocol-based therapy like pharmacokinetic dosing and IV to PO conversions. They adjust doses for parameters such as renal or hepatic dysfunction with appropriate documentation.
Pharmacists assure timely medication order processing, participate in order verification, and resolve problem orders. They support department staffing by flexing to other areas and engage in performance improvement efforts to strengthen systems and processes. They report medication incidents in Patient Safety Net and participate in reviews to establish safe systems.
They participate in medication reconciliation at admission, transfer, and discharge, providing patient education as needed. PAC pharmacists share expertise through educational presentations for pharmacy and non-pharmacy staff. They support residency programs, clerkships, and interns by precepting rotations, offering feedback, and collaborating on projects.
Requirements
- Bachelor's Degree in Pharmacy from an accredited college or university
- Licensed Pharmacist - State Board of Pharmacy in state of primary practice site
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
- Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA) certification as necessary
- Pediatric Advanced Life Support (PALS) and/or Neonatal Resuscitation Program (NRP) certification as necessary
- Completion of an accredited pharmacy post-graduate training program (PGY-1 and/or PGY-2 residency program) preferred
Responsibilities
- Actively participates on the multidisciplinary team by attending patient care rounds and providing patient-focused pharmaceutical care
- Completes patient-specific medication profile reviews and executes protocol-based drug therapy including pharmacokinetic dosing and IV to PO conversions
- Assures timely medication order processing by participating in order entry, verification, and resolving problem orders
- Participates in medication reconciliation at admission, transfer, and discharge and provides patient education
- Continually reports medication incidents and adverse drug reactions in Patient Safety Net and participates in reviews
- Shares pharmaceutical expertise by coordinating educational presentations or in-services for staff
- Supports pharmacy residency programs, clerkship students, and interns by precepting rotations and providing mentorship
