About this role
The Sloane Hospital in Kent is part of Circle Health Group, Britain's leading provider of independent healthcare with a nationwide network of hospitals and clinics. We have an opportunity for a Clinical Pharmacist to join the Pharmacy Department team. This is a bank role working on an 'as and when required' basis to supplement staffing during busy periods and cover absences.
Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation. Provide tailored advice to patients on treatment plans, including side effects, compliance, and lifestyle considerations. Support safe prescribing through reconciliation, clinical screening, and dose adjustment.
Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements. Promote safe and cost-effective use of medicines by supporting adherence to local and national policies. Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability.
Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance.
