AnewHealth is a leading pharmacy care management company specializing in complex chronic needs for patients at home. The Clinical Pharmacist assumes responsibility for pharmacotherapy outcomes using knowledge, innovative technology, and medication packaging and distribution. This role supports better outcomes for patients and healthcare organizations across all 50 states.
Serves as point of contact for patients, families, pharmacists, clients, and providers to address medication-related problems, health issues, and pharmacotherapy questions. Explains care plans, medication use, side effects, and contraindications verbally or via records in proprietary software. Ensures proper documentation of patient care decisions, coding, and billing.
Oversees accurate, efficient medication distribution, compounding, packaging, and labeling per policy and legal requirements. Supervises Pharmacy Technicians in reconciliation, order entry, inventory, storage, and compliance. Provides review, preparation, and approval for prescription orders to ensure correct drug, dosage, and dispensing.
Uses professional judgment for recommendations to care providers enhancing patient outcomes and safety. Ensures compliance with state, federal laws, and internal policies, reporting any violations. Embodies AnewHealth’s Core Values in all interactions while adapting to departmental needs.
Requirements
- Licensed pharmacist with knowledge of pharmacotherapy outcomes
- Proficiency in proprietary pharmacy software for documentation, billing, and patient profiles
- Experience addressing medication-related problems (MRPs) and providing pharmaceutical care recommendations
- Skills in medication compounding, packaging, labeling, and distribution compliance
- Ability to oversee pharmacy technicians in reconciliation, inventory, and order entry
- Familiarity with state and federal pharmacy laws, HIPAA, and regulatory guidelines
- Professional judgment for assessing new requests, refills, and care plan adjustments
Responsibilities
- Serves as point of contact for patients, families, other pharmacists, clients, and healthcare providers to address medication-related problems (MRPs), health issues, and pharmacotherapy needs
- Ensures accurate, efficient, timely, and compliant medication distribution and fulfillment using proprietary software
- Plans, implements, and maintains procedures for assessing, compounding, packaging, and labeling medications according to policy and legal requirements
- Oversees Pharmacy Technicians to ensure medication reconciliation, physician order entry, inventory management, storage, security, labeling, and compliance
- Provides timely review, preparation/compounding, and final approval for prescription orders ensuring correct drug, dosage, directions, quantity, and packaging
- Uses professional judgment to assess medication requests and refills, making recommendations to care providers to enhance patient outcomes and safety
- Ensures compliance with state and federal laws, regulations, and internal policies; reports and rectifies incidents of quality assurance, HIPAA violations, or fraud
