About this role
The Pharmacy Specialist delivers high-quality, cost-effective pharmaceutical care to specific patient populations within the assigned scope of practice. Focuses on Comprehensive Medication Management (CMM) for patients referred by INTEGRIS Medical Group primary care physicians and employees. Adheres to National Patient Safety Goals based on patient contact level.
Provides concise, applicable, and timely drug information responses to INTEGRIS Health professionals. Identifies drug use issues for policy development and coordinates concurrent drug use evaluation with staff pharmacists. Assists in assessing, managing, preventing, and reporting adverse drug events while tracking trends for education.
Prepares drug monographs for Pharmacy and Therapeutics Subcommittee review and oversees formulary medications availability. For CMM, elicits medication history, assesses regimens for efficacy, safety, interactions, adherence, and costs. Addresses medication problems through education, changes per protocols, or physician recommendations.
Communicates effectively with patients, physicians, care coordinators, and team members while educating on medications. Documents all CMM visits timely in the electronic health record. Reports to the assigned Manager in a collaborative healthcare environment.
Requirements
- Post graduate year one (PGY1) residency or equivalent experience
- Graduation from a program in Pharmacy from an accredited school of pharmacy
- Advanced degree in pharmacy and/or a pharmacy residency or fellowship or equivalent experience
- Current Oklahoma Pharmacy license
Responsibilities
- Provides concise, applicable, and timely responses to requests for drug information from healthcare professionals affiliated with INTEGRIS Health
- Identifies drug use issues requiring policy development
- Coordinates the concurrent drug use evaluation program involving the staff pharmacists
- Assists in the assessing, managing, preventing, and reporting of adverse drug events
- Tracks and trends ADRs for potential educational opportunities/drug policy issues
- Prepares and presents drug and therapeutic class monographs for formulary review
- Assesses medication regimen for efficacy, safety, clinically-relevant drug interactions, medication adherence, and costs of therapy
- Documents all CMM visits in the electronic health record in a timely manner
