Requirements
- Licensed pharmacist with expertise in population health management
- Ability to design, implement, and oversee pharmaceutical programs
- Proficiency in analyzing population health data for medication trends and interventions
- Knowledge of medication management protocols, adherence strategies, and safety best practices
- Experience developing educational materials for patient medication adherence and outcomes
- Familiarity with quality improvement initiatives in pharmaceutical services
- Understanding of regulatory standards and compliance in pharmaceutical services
- Leadership skills to guide pharmacy teams on population health principles
Responsibilities
- Design and develop pharmaceutical programs aligned with population health goals and healthcare policies
- Collaborate with healthcare providers, including physicians, nurses, and other specialists, to optimize medication therapy plans for specific patient populations
- Analyze population health data to identify medication-related trends, gaps in care, and opportunities for intervention
- Develop protocols for medication management, adherence, and safety, ensuring best practices are followed
- Create educational materials and initiatives to enhance patient understanding of medication usage, adherence, and health outcomes
- Implement quality improvement initiatives related to pharmaceutical services within the population health framework
- Ensure compliance with regulatory standards, laws, and accreditation requirements related to pharmaceutical services
- Provide leadership to the pharmacy team, offering guidance, training, and support to enhance their understanding of population health principles and practices
- Establish and monitor key performance indicators (KPIs) to evaluate the effectiveness of pharmaceutical programs and interventions
