Requirements

Licensed pharmacist with expertise in population health management
Ability to design, implement, and oversee pharmaceutical programs
Proficiency in analyzing population health data for medication trends and interventions
Knowledge of medication management protocols, adherence strategies, and safety best practices
Experience developing educational materials for patient medication adherence and outcomes
Familiarity with quality improvement initiatives in pharmaceutical services
Understanding of regulatory standards and compliance in pharmaceutical services
Leadership skills to guide pharmacy teams on population health principles

Responsibilities

Design and develop pharmaceutical programs aligned with population health goals and healthcare policies
Collaborate with healthcare providers, including physicians, nurses, and other specialists, to optimize medication therapy plans for specific patient populations
Analyze population health data to identify medication-related trends, gaps in care, and opportunities for intervention
Develop protocols for medication management, adherence, and safety, ensuring best practices are followed
Create educational materials and initiatives to enhance patient understanding of medication usage, adherence, and health outcomes
Implement quality improvement initiatives related to pharmaceutical services within the population health framework
Ensure compliance with regulatory standards, laws, and accreditation requirements related to pharmaceutical services
Provide leadership to the pharmacy team, offering guidance, training, and support to enhance their understanding of population health principles and practices
Establish and monitor key performance indicators (KPIs) to evaluate the effectiveness of pharmaceutical programs and interventions
