About this role

The Clinical Pharmacy Specialist supports patient care responsibilities for specialty pharmacy patients, focusing on oncology. Serves as a liaison between Specialty pharmacy staff and the Assistant Director of Pharmacy. Delivers comprehensive services across access, delivery, and innovative clinical care for patients receiving specialty medications.

Applies clinical knowledge and expertise of specialty disease state management to provide patient assessment, medication review and reconciliation, patient education, and regular follow-up. Develops individualized patient care plans and performs therapy management baseline and periodic reassessment prior to medication dispensation. Includes adherence monitoring, drug interaction assessment, side effect management, and comprehensive medication reviews.

Supports providers in selecting appropriate specialty medications, responds to drug information requests, and writes insurance appeal letters. Ensures optimal effectiveness and safety of dispensed medications while providing functional supervision of technical staff. Oversees daily operations, maintains productivity, and contributes to accreditation and quality improvement initiatives.

Develops and presents educational topics, staff development programs, and medication protocols. Maintains clinical pharmacy rotations for students and residents, mentors scholarly activities, and participates in the Quality Management Committee. Assists with policy development, performance evaluations, training, and quarterly performance improvement reports.

Requirements

  • Clinical knowledge and expertise of specialty disease state management, particularly oncology
  • Ability to perform patient assessment, medication reconciliation, adherence monitoring, and side effect management
  • Proficiency in comprehensive medication reviews, patient counseling, and education on specialty medications
  • Working knowledge of all State and Federal laws and regulations affecting the practice of pharmacy
  • Experience supporting insurance appeals and responding to drug information requests for specialty medications
  • Skills in therapy management, drug interaction assessment, and patient follow-up
  • Ability to develop care plans and conduct baseline/periodic reassessments prior to dispensation
  • Oncology pharmacy expertise for optimizing patient drug therapy and outcomes

Responsibilities

  • Develop individualized patient care plans and perform therapy management baseline and periodic reassessment for every enrolled patient
  • Support providers in selecting appropriate specialty medications, respond to patient-specific drug information requests, and write insurance coverage appeal letters
  • Ensure optimal effectiveness and safety of medications dispensed and provide accurate, clear, and timely drug information to healthcare teams and patients
  • Provide functional supervision of technical staff, including completion of job assignments and adherence to policies and procedures
  • Manage time efficiently and oversee daily operations to maintain high productivity of Specialty Pharmacy Services staff
  • Contribute to accreditation maintenance, renewal, quality improvement initiatives, and metrics
  • Develop and present educational topics, staff development programs, and medication protocols; serve as clinical resource to healthcare team
  • Assist Assistant Director of Pharmacy with policy development, performance evaluations, training, and quarterly performance improvement reports
